Skilled Nursing Facility (SNF)
Our Skilled Nursing Facility (SNF) at Home programs are designed to offer patients a choice of receiving their post-acute medical rehabilitation in the safety and comfort of their own home. These programs allow the home to be taken to the next level clinically, while still offering patient focused rehab sessions.
These programs are designed to offer patients a choice of receiving their post-acute medical rehabilitation, in the safety and comfort of their own home. These programs allow home to be taken to the next level clinically while still offering patient focused rehab sessions:
- By being at home, the depression and isolation patients can feel can be avoided
- The home environment provides an atmosphere of wellness increasing patient control and motivation
- The patient and family are able to develop effective coping strategies to face the necessary changes in life with the support of the Pathways team in their own home
- Family routines and interactions combined with in-home support of the Pathways team help to keep the family both physically and emotionally intact. The stress of commutes to a facility are avoided
- Local resources can be accessed early in the rehabilitation process facilitating community reintegration
- Therapy occurs in the patient's living environment eliminating time lost to set backs often experienced upon discharge from institutional based programs
Orthopedic/Spine
This program is designed for patients who are preparing for, or recently had, a joint replacement or other orthopedic surgery. Our program can avoid or shorten a post-acute facility stay after a surgery. Our ortho/spine care plan can be tailored to be surgeon specific.
Cardiac Rehab
This program is designed to provide home health services to your patients in the comfort of their home. Our health care team consisting of nurses, physical, occupational therapists, social workers work closely with each patient to provide a customized plan of care for each patient.
Pulmonary Rehab
This program is designed to help patients better manage symptoms, improve ability to exercise, breathe easier and improve quality of life; allowing patients to safely re-enter community and thrive in their home.
Neurological Rehab
This program is designed to help patients improve function and reduce symptoms resulting from diseases, injury, or disorders of the nervous system to improve safety and Independence in the home.
Who Pays for Home Health Care
Certified home health care services are ordered by a physician, require skilled care, and is typically paid for by Medicare or other health insurances plans.
